Published by IBM in June 2026, this study surveyed CIOs and CTOs on the gap between how fast enterprise AI deployment is scaling and how much control technology leaders actually have over the systems they're accountable for. Two-thirds of surveyed CIOs and CTOs report being held responsible for AI systems they don't fully control, and 77% say AI adoption is already outpacing their organization's governance capabilities.
The report also found a visibility problem underneath the governance one: 70% of executives say teams across the business are deploying AI tools faster than IT can track them. Despite 80% reporting CEO-driven AI transformation mandates, only 11% feel ready for the scale of AI agent deployment they expect over the next year, a gap that echoes similar employee-adoption concerns IBM has raised in its separate CEO-focused research.
